The June 1, 2008 rate changes and contributions have been updated in this latest Contribution Report spreadsheet.

Note: This report is only for the period post June 1, 2008. The Contractor Contribution Report June 1, 2007 (v7) below must still be used for all reports prior to June 1, 2008.

New Additions to the form

Apprentice Hours

There is a new addition to the 0-3 Year Apprentices Section. 0-3 Year Apprentice hours now need to be separated into Worked hours and Education-Training (school) hours.

Contractor contributions for 0-3 Year Apprentices now apply only to Worked Hours. Contractor contributions no longer apply to Education-Training hours. Education-Training hours need to be reported separately to properly calculate working dues, which continue to apply to these hours, without contribution deductions.

Please make sure to fill out the top portion of the form: Contractor Name, Address, City State and Contractor Numbers (located to the right of Contractor Name). This will expedite the processing of your contribution information.
