Welfare Fund - Technical Engineering Division

Required Covered Employee Information

Any of the following events requires our Covered Employee and/or dependents to send the proper associated documentation to the Welfare Fund office:

Marriage (a) Copy of Marriage certificate
(b) New Beneficiary Card
(c) Coordination of Benefits form
Divorce or Legal Separation (a) Copy Divorce decree or Court Order
(b) New Beneficiary Card
Birth (a) Copy of Child’s birth Certificate
(b) Copy of Social Security Card
(c) New Beneficiary Card
Termination of Spouse’s primary insurance Letter of Termination from Primary insurance
New employement of Spouse Coordination of Benefits form
Dependent child reaching the age of 19 Copy of full-time student verification letter from an accredited college
Change of address E-mail to maguirre@ualocal130welfare.com
